ADDITIONAL INFORMATION The following services must be provided, directly or through contract: 1) primary health care and substance abuse services at locations accessible to homeless individuals; 2) 24-hour emergency primary health and substance abuse service to homeless individuals; 3) referral of homeless individuals as appropriate to medical facilities for necessary hospital services; 4) referral of homeless individuals who are mentally ill to entities that provide mental health services, unless the applicant will provide such services; 5) outreach services to inform homeless individuals of the availability of primary health and substance abuse services; 6) aid to homeless individuals in establishing eligibility for assistance and in obtaining services under entitlement programs; and 7) podiatry, dental (including dentures), and vision services are supplemental services and may be provided where medically necessary, to the extent that the level of delivery of the required services is not diminished. Independent Choices is designed to foster the development of consumer-based services for people of all ages with chronic disabilities. Consumer-directed care is based on the principle that individuals who are disabled or their chosen advocates, not funders or providers of services, should have the primary responsibility for making decisions regarding the assistance they receive. Independent Choices will fund demonstration projects that test new approaches for organizing and financing programs that substantially increase consumer direction and control. Research that generates new knowledge regarding quality and costs of consumer-directed services will be funded also. ADDITIONAL INFORMATION The consumer-directed model has at least four important dimensions: 1) the ability of the consumer to control and direct the delivery of services, i.e., the consumer is the primary decision maker, assuming responsibility for controlling virtually all aspects of care or delegating responsibility to other individuals; 2) the availability of a variety of service delivery options, i.e., the consumer has meaningful choices among a range of viable service options and also has the ability to create new options outside the traditional system; 3) the availability of appropriate information and support, i.e., the consumer is informed to the greatest extent possible about service options and the personal, legal, and financial issues associated with each; and 4) the ability of the consumer to participate in the policy-making process, e.g., in the design of service delivery systems or in the service allocation process. OTHER LIMITATIONS Public and private organizations, agencies, or institutions are eligible to apply under this program as long as their proposed program or research is for charitable purposes.
