Syphilis is linked to morbidity and mortality through congenital syphilis, and through its cofactor role in HIV transmission. Therefore, effective, innovative, community-based approaches to syphilis prevention may improve both HIV prevention and overall health in the communities where syphilis is most prevalent. The program is divided into two parts: Phase I - preparation for innovative approaches to syphilis control and prevention; and Phase II - implementation of interventions and evaluation of impact on population. This announcement focuses on Phase I. Four key research components will be addressed: 1) innovative uses of research methods and findings about syphilis transmission to direct prevention strategies; 2) development and pilot test of a programmatic syphilis intervention; 3) development of partnerships with communities affected by syphilis; and 4) development of a surveillance activity to be used in evaluating the efficacy and cost-effectiveness syphilis prevention efforts. Project goals include: 1) establishment or strengthening of partnerships among research institutions, health departments, and at least one community-based organization; 2) epidemiological analysis of the transmission of syphilis; 3) testing and assessment of a programmatic intervention intended to lower syphilis transmission in a defined area; 4) implementation of a sensitive surveillance system (used to follow trends in syphilis transmission) and collection of one year of baseline data; and 5) implementation of a management information system to track activities and costs of syphilis prevention activities.
