The goal is to promote collaborative epidemiologic and prevention effectiveness research on the most serious health threats facing urban residents, thus developing information for public health planning and for improving the health of citizens. Preventive interventions may be directed at the high rates of disease and injury of residents of urban neighborhoods, including sexually transmitted diseases (STDs), AIDS and tuberculosis. Population laboratories will be established to produce information useful in health policy decisions and planning. The specific objectives of the population laboratory include the following: 1) to assess the availability and capacity of existing prevention programs; 2) to foster the development of collaborative relationships among the population laboratory, CDC, and the State and local health departments in order to focus collective expertise on high priority urban health problems; 3) to develop and implement organizational and sociological intervention studies to optimize effectiveness in prevention programs; and 4) to develop a multidisciplinary approach to prevention programs and to develop, test, evaluate and disseminate model programs to enhance health promotion and disease and injury prevention in various settings and populations. Fund recipients are responsible for: 1) the establishment and operation of a demonstration population laboratory, including its Executive Board, in a defined population in a geographically defined urban area; 2) proposing and conducting multidisciplinary research dedicated to improving the health and well-being of urban populations; 3) managing, analyzing, and interpreting data from population laboratory projects, and publishing and disseminating important public health information; 4) Monitoring and evaluating scientific and operational accomplishments of the population laboratory; 5) documenting findings in the scientific literature; 6) evaluating specific interventions (programs) to address typical urban health problems researched by the laboratory; and 7) foster the development of prevention programs that cut across health issues that affect common populations. Collaboration with health care providers, especially managed care organizations, is encouraged. Applicants must provide evidence of: 1) working relationships with partner organizations and community leaders; 2) supplemental technical and financial assistance from other "partner organizations"; 3) expertise in research related to urban and minority health issues or a planned process for developing such expertise in a short timeframe; and 4) plans for core activities, demonstration projects, or collaborations/collaborative projects with State/local health departments and academic institutions. All applicants compete for Part I of the STD/HIV PTCs program: Clinical Services Training. Part II (Health Behavior Training, 4 awards) and Part III (Partner Counseling Training, 4 awards) are elective, and are only open to recipients of funds under Part I. The purpose of these agreements is to support innovative health professional training programs in integrated STD and HIV client management within a national network of STD/HIV PTCs to achieve a comprehensive prevention strategy, including clinical, health behavioral, and partner counseling interventions. The aims of the cooperative agreement are therefore to: 1) augment the capacity to reach needy minority populations and improve health benefits for women, infants, and adolescents; 2) facilitate integrated prevention efforts; 3) support a comprehensive disease prevention strategy through clinic-based and community-based activities; 4) anticipate the emerging health care reform demand to maintain an efficient national work force; and 5) prepare, through experiential activities, persons who are studying to be health practitioners. Training will be accomplished by: 1) establishing regional training centers in order to participate in a national network of training in the procedures and guidelines for STD and HIV client management; 2) offering clinic-based and community-based training experience with clients in a public health setting; 3) developing the capacity in communities to work with health departments in the planning, production, delivery, and evaluation of training; 4) using advances in communications technology in innovative distance learning methodologies; and 5) designing analytic methods for educationally relevant and cost-effective training. Any materials developed with Public Health Service (PHS) funds will be subject to a nonexclusive, irrevocable, royalty-free license to the government to use and to authorize others to use for government purposes.
